We are looking for a Mental Health Worker to join our Youth Mental Health Services team in Tennant Creek, NT.
The Youth Mental Health Worker is responsible for providing assessment and casework of young people experiencing, who are at risk of, or who may have a mental illness. The position works collaboratively with a Senior Mental Health Worker or Lead Practitioner and often in consultation with other mental health providers to provide services to young people, their families and carers. This position has a particular focus on implementing and reviewing the care plan, assessment of family needs, joint engagement of young person and family, addressing the psycho-social needs of the family and mediation if required. This position works to create and maintain a collaborative and consistent care model, which supports recovery and maintenance and seeks input from the young person, their significant others and other relevant service providers.
The Youth Mental Health Worker in collaboration with other YAMHS team members also undertakes community-based activities that promote social, emotional health and wellbeing.
The Youth and Mental Health Service (YAMHS) is delivered by CatholicCare NT (CCNT) to provide a service to young people at risk of, or who are experiencing mental illness. YAMHS is designed to increase service access to improve the chances of recovery and longer-term social and emotional health and wellbeing.
YAMHS engages with young people, their family and carers to improve their knowledge and understanding of the impact of mental illness. YAMHS does this by working collaboratively with the young person, family, carers and stakeholders and by providing assessment, casework and specialist mental health liaison and community education.
